Become a part of our caring community The Behavioral Health Care Manager (Care Manager, Telephonic Behavior Health 2) in a telephonic environment, assesses and evaluates members' needs and requirements. This is done to achieve and/or maintain optimal wellness state.

You will guide members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Behavioral Health Care Manager’s work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

The Behavioral Health Care Manager (Care Manager, Telephonic Behavior Health 2) employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues. Identify and resolve barriers that hinder effective care.

Ensure patients are progressing towards desired outcomes by continuously monitoring patient care through assessments and evaluations. May create member care plans. Understand department, segment, and organizational strategy and operating objectives, including their linkages to related areas.

Makes decisions regarding own work methods, occasionally in ambiguous situations, and require minimal direction and receives guidance where needed. Follow established guidelines/procedures. Use your skills to make an impact Applicants must meet one (1) of the following: An active, unrestricted Registered Nurse (RN) license in the state of South Carolina.

An active, unrestricted social work license in South Carolina in one of the following disciplines: Licensed Independent Social Worker – Clinical Practice (LISW-CP) , Licensed Master Social Worker (LMSW) or Licensed Professional Counselor (LPC).

Applicants must meet all of the following: Must reside in the state of South Carolina. Three (3) or more years of clinical acute care experience in a behavioral health setting. One (1) or more years of care coordination/care management experience.

Experience with serious mental illness specialty population. Comprehensive knowledge of Microsoft Office applications including Excel, and Outlook. This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program.

If selected for this role, you will be required to be screened for TB. This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance.

Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher. Preferred Qualifications Bachelor’s degree in nursing (BSN).

Experience with case management, discharge planning and patient education for adult acute care. Managed care experience. Certified Case Manager (CCM). Additional Information Workstyle: This is a remote position with some travel. Travel: Up to 25% field- based interactions with members and/or their families, community partners and care teams.

May need to attend occasional onsite meetings in Humana’s Columbia, SC office. Mileage Reimbursement for Travel: Mileage reimbursement is provided for work-related travel. Eligible mileage includes travel from your home to your first work location, travel between client or assignment locations during the workday, and travel from your final work location back to your home.

Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required. Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.

The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $59,300 - $80,900 per year This job is eligible for a bonus incentive plan.
